Pdf Summary
The Home Centered Care Institute (HCCI) hosted a pre-summit event on November 8, 2021, at the C-TAC National Summit on Advanced Illness Care, focusing on the Florida House Call Project. The discussion featured a panel comprising Jennifer Ayrey, Madetric Wood, Aaron Yao, and Melissa Singleton, amongst others. Key objectives of the session included sharing insights from the Florida House Call Project, evaluating research findings on awareness and perceptions of home-based primary care (HBPC), and sharing compelling stories from the field.

The project aims to broaden the reach of home-centered care in Florida, boost demand and capacity for such services, and strengthen the workforce through research and enhanced awareness. The HCCI encourages the use of media and public forums to spread awareness and education about HBPC. A significant cohort consisting of physicians, nurse practitioners, and business/operations managers actively champions these efforts across many counties in Florida.

Surveys conducted among Florida residents and healthcare providers reveal a predominantly positive perception of HBPC. They highlight the importance of mobility issues and the preference for current primary care providers to be involved in referrals. The session also presented detailed statistics on the current supply, patient demographics, and demand distribution of HBPC within Florida.

The event featured an array of stories from HBPC providers, illustrating the profound impact of house calls on patient care, from reducing unnecessary hospital visits to improving the quality of life for homebound patients. The session aimed to inspire and recruit more advocates for home-centered care by showcasing these success stories and forwarding the "I House Calls" campaign initiative.

HCCI also announced upcoming workshops and webinars as part of their continuous efforts to educate and support HBPC providers. The session concluded by inviting participants to share their stories on the HCCI website and stay connected for further resources and educational opportunities.
