The Florida House Call Project aims to raise awareness and accessibility of Home-Based Primary Care (HBPC) in Florida, a state with a high number of Medicare patients suffering from multiple chronic illnesses. These Champions shared their experiences and insights at a virtual pre-summit session held at the C-TAC National Summit on November 8, 2021.<br /><br />The session's main objectives included discussing key lessons from the project, evaluating research findings related to HBPC awareness and perceptions, pinpointing areas with care gaps and unmet needs, and sharing inspiring personal stories that highlight the impact of house calls.<br /><br />The agenda for the pre-summit session was as follows:<br />- **3:00-3:10 p.m. ET**: Welcome and introductions.<br />- **3:10-3:25 p.m. ET**: Presentation by Home-Centered Care Champions on their activities, outcomes, and an interactive Q&A session.<br />- **3:25-3:40 p.m. ET**: Discussion on the awareness and perception of HBPC in Florida, based on surveys of healthcare consumers and providers, followed by a Q&A session.<br />- **3:40-3:55 p.m. ET**: First part of the supply and demand analysis of HBPC in Florida.<br />- **3:55-4:00 p.m. ET**: Break.<br />- **4:00-4:30 p.m. ET**: Continuation of the supply and demand analysis, including an exploration of interactive dashboards and a Q&A session.<br />- **4:30-4:55 p.m. ET**: Stories segment titled "I heart House Calls".<br />- **4:55-5:00 p.m. ET**: Wrap-up and adjournment.<br /><br />The session highlighted the critical role of community-based models and the positive outcomes of engaging local champions in enhancing HBPC awareness and access in Florida.
