ILHCP One-Day In-Person Workshop
Description
We aim to improve learners' knowledge, competence, performance, and outcomes by training and equipping them to collectively enroll and serve 3,000 new homebound or home-limited Illinois residents in their homes by 2024, as well as to serve as leaders and champions of home-based primary care using HCCI education curriculum, resources, tools, and market analysis. From research studies and such real-world trials as the CMS Independence at Home demonstration, we know that innovative home-centered care models effectively address both patient and provider needs in the current healthcare environment. Learners of this activity will raise awareness within their communities and peer groups regarding HBPC as an alternative model of care for the frail, complex, chronically ill, and disabled aging population resulting in improved patient outcomes, improved patient satisfaction, reduced costs, and improved experience for physicians and other practitioners.
Objectives
1. Use the Media Training Kit to elevate public and provider awareness of home-based primary care, including its benefits to patients, families and caregivers, providers, and payers.
2. Describe the prevalence of house call providers and practices in Illinois, assess the current utilization of home-based primary care (HBPC) services, and identify geographic areas where the need for HBPC is not being met.
3. Discuss obstacles or burdens faced by home-based primary care providers and their teams and apply mitigation strategies.
4. Utilize innovative instructional methods and HCCI resources, tools, and curriculum to lead or facilitate education at the local and state levels for providers and practice/operations staff
Intended Audience
This workshop is intended for physicians, advanced practice providers (NPs/PAs), nurses (RNs, LPNs), practice managers/administrators, medical assistants, and other practice operations staff participating in HCCI’s Illinois House Call Project.
