Advanced Applications of Home-Based Primary Care Workshop
Description
Designed for experienced home-based primary care (HBPC) providers and practice staff, this two-day workshop will prepare and equip learners to succeed in value-based payment arrangements. This activity is also ideal for those who have completed the Essential Elements of Home-Based Primary Care™ workshop and are ready to continue their learning. Sessions present actionable strategies with a value-based care lens in the following areas: required staffing, infrastructure, and data; patient identification; cost-effective management of medications and conditions commonly found in HBPC patients (e.g., dementia, CHF); quality measurement and reducing gaps in care; risk adjustment models, including Hierarchical Condition Category (HCC) coding; and demonstrating the value of HBPC when negotiating with payers. Also available is hands-on training in HBPC procedures including wound care, gastrostomy and tracheostomy tube changes, and joint injections. 

NEW for the December 1-2, 2022 workshop! 2-hour, hands-on Point-of-Care Ultrasound (POCUS) session

The advent of POCUS has the potential to advance the delivery of care to homebound patients by augmenting physical exams to enhance clinical decision making in patients’ homes and facilitate medical decision-making.

This session will review POCUS fundamentals, including basic physics and instrumentation, and will provide hands-on experience using common, case-based geriatric conditions. Attendees will interact with standardized patients to practice scanning and image interpretation.


Intended Audience
This workshop is intended for physicians, advanced practice providers (NPs, PAs), nurses (RNs, LPNs), practice managers/administrators, social workers, residents/fellows, medical assistants, and other practice operations staff. 
Objectives

Upon completion of this workshop, learners will be able to: 

  • Discuss value-based payment opportunities currently available to HBPC programs, including strategies for patient identification, quality measurement, contract negotiation, and establishing key partnerships.
  • Apply recommended practices for staffing and productivity, efficiency, telehealth, risk stratification, and coordination with other providers.
  • Provide high-quality, cost-effective patient care and successfully manage conditions of specific concerns in value-based payment arrangements, including dementia, congestive heart failure, and polypharmacy.
  • Manage transitions of care for homebound patients within the domains of accountability, communication, assessing needs and goals, medication management, and safety.
  • Perform common procedures typical in HBPC, including tracheostomy tube exchange, gastrostomy tube removal and replacement, wound care, knee joint aspiration and injection, and point-of-care ultrasound.

Room Rates (excluding taxes):
Standard rooms available are a King Suite. Reservations using HCCI's discount rate of $159 per room, per night can be made for Wednesday, November 30th through Friday, December 2nd.

Reservation Cut-off Date:
HCCI’s discounted group rates are available on a first-come, first-served basis through Friday, November 18, 2022. Attendees are encouraged to reserve their rooms early, as the block may be filled before November 18.

Parking:
Complimentary parking will be offered to all registered hotel guests. Guests must register their vehicle immediately at check-in to avoid receiving parking fines.

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of PeerPoint Medical Education Institute and the Home Centered Care Institute. PeerPoint Medical Education Institute is accredited by the ACCME to provide continuing medical education for physicians.

Cancellation Policy
Maximum Workshop Enrollment 
Workshop enrollment is limited to 40 learners per offering. If paying by check, your registration is not considered final until payment has been received and your registration confirmed by HCCI.
 
Cancellation & Refunds 
HCCI reserves the right to cancel a Workshop at any time for any reason. If HCCI cancels a Workshop, you will have the option to either a) transfer tuition paid to another HCCI Workshop or b) receive a refund.You must inform HCCI of your choice of either the transfer or refund within two (2) weeks of the Workshop cancellation. If no such notification is received, HCCI will issue a refund.
 
If you wish to cancel your registration you must send notice of cancellation via e-mail to education@hccinstitute.org. Notices received prior to 5:00 pm CST two weeks prior to the first day of the Workshop will be refunded the amount of the tuition paid less a $75 administrative fee. In lieu of a refund, the full amount of the tuition paid may be applied to a future HCCI Workshop. The request to apply tuition credit to a future Workshop must be made in writing by e-mail within two weeks of the cancellation or a refund will be issued.
   
Cancellation requests submitted after the deadline noted above, will not receive a refund. No-shows will not be eligible for a refund or transfer of tuition.  

Other 
Requests to transfer your registration to another person will be considered on a case-by-case basis. Transfer requests must be made in writing no later than 5:00 pm CST one week before the first day of the Workshop. The Workshop is designed as a comprehensive two-day activity, and enrollment may not be “split” between participants. Likewise, there is no single-day tuition. 

HCCI is not liable or responsible for any travel, lodging or work expenses incurred in the event of a Workshop cancellation that is initiated by either HCCI or the learner.
