Essential Elements of Home-Based Primary Care Workshop
Description
The Essential Elements of Home-Based Primary Care™ will be presented in-person in San Diego, California on November 10-11, 2022. This workshop is designed for learners who are considering or who are relatively new to offering home-based primary care (HBPC) services for patients with complex medical conditions. In this highly interactive activity, learners will enhance their knowledge and skills through case-based sessions focusing on clinical and economic factors unique to HBPC, including patient assessment; prognostication and advance care planning; medication management; acute/urgent care; technology useful in enhancing patient care and productivity; supporting caregivers; cultural competence; staffing, operations and process workflows; budgeting and revenue cycle management; coding and billing; and provider safety and self-care.
Intended Audience
This workshop is intended for physicians, advanced practice providers (NPs/PAs), nurses (RNs, LPNs), practice managers/administrators, social workers, residents/fellows, medical assistants, and other practice operations staff.
Objectives

Upon completion of the workshop, learners will:

1. Describe the foundational principles of HBPC, including practice settings and administration, clinical care models, patient demographics, and characteristics of successful HBPC providers.  

2. Apply the 4Ms in the Care of Older Adults to the HBPC population: home-limited patients with multiple co-morbidities. 

3. Apply recommended approaches to various aspects of clinical care for homebound patients including medication management, mental health concerns, prognostication, and acute/urgent care. 

4. Provide optimal patient care in a simulated home setting and demonstrate appropriate coding and documentation for the visit. 

5. Recognize caregiver burnout and provide coping strategies and other resources. 

6. Examine and discuss the business models, economic drivers, and quality indicators for HBPC and their impact on care delivery. 

7. Compare various staffing models for HBPC and examine opportunities for improving workflows and efficiency.

Parking:
Attendees registered within the hotel block will qualify for $24 overnight parking. Daily parking at the DoubleTree by Hilton San Diego Mission Valley will be at $10/day.

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of PeerPoint Medical Education Institute and the Home Centered Care Institute. PeerPoint Medical Education Institute is accredited by the ACCME to provide continuing medical education for physicians.

PeerPoint Medical Education Institute designates this live activity for a maximum of 13 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Cancellation Policy
Maximum Workshop Enrollment 
Workshop enrollment is limited to 40 learners per offering. If paying by check, your registration is not considered final until payment has been received and your registration confirmed by HCCI.
 
Cancellation & Refunds 
HCCI reserves the right to cancel a Workshop at any time for any reason. If HCCI cancels a Workshop, you will have the option to either a) transfer tuition paid to another HCCI Workshop or b) receive a refund.You must inform HCCI of your choice of either the transfer or refund within two (2) weeks of the Workshop cancellation. If no such notification is received, HCCI will issue a refund.
 
If you wish to cancel your registration you must send notice of cancellation via e-mail to education@hccinstitute.org. Notices received prior to 5:00 pm CST two weeks prior to the first day of the Workshop will be refunded the amount of the tuition paid less a $75 administrative fee. In lieu of a refund, the full amount of the tuition paid may be applied to a future HCCI Workshop. The request to apply tuition credit to a future Workshop must be made in writing by e-mail within two weeks of the cancellation or a refund will be issued.
   
Cancellation requests submitted after the deadline noted above, will not receive a refund. No-shows will not be eligible for a refund or transfer of tuition.  

Other 
Requests to transfer your registration to another person will be considered on a case-by-case basis. Transfer requests must be made in writing no later than 5:00 pm CST one week before the first day of the Workshop. The Workshop is designed as a comprehensive two-day activity, and enrollment may not be “split” between participants. Likewise, there is no single-day tuition. 

HCCI is not liable or responsible for any travel, lodging or work expenses incurred in the event of a Workshop cancellation that is initiated by either HCCI or the learner.
