Description
House call practices face many challenges when trying to survive financially in the current fee-for-service world. Chances are your practice is not taking full advantage of reimbursement opportunities for services provided beyond the standard evaluation and management (E/M) codes. 
Objectives
In this course, you will learn about coding opportunities and documentation requirements associated with Transitional Care Management, Chronic Care Management, Advanced Care Planning, and Prolonged Services - thus, providing you the insight needed to maximize your revenue potential without increasing your workload. Upon completion, you will be able to do the following:
  1. Describe the regulatory compliance requirements for Transitional Care Management, Chronic Care Services, Advanced Care Planning, and Prolonged Services.
  2. Identify implications at the practice and the individual provider level that must be considered when implementing the described codes and services.

30 minutes
In order to earn credit for participation in this activity, the learner must complete a 5-question post-test with a passing score of 80% or better with unlimited attempts.

This course is intended for home-based primary care providers, as well as coders, practice managers, and operations staff.
