To address the challenges of decreased reimbursement for the HBPC field in 2021, HCCI is making this course available for free through April 30, 2021. The course is designed to support HBPC providers and practices, particularly those operating in fee-for-service looking to maximize revenue and enhance sustainability. The course is available at no cost for this limited time through the generous support of The John A Hartford Foundation.

Description
There are likely many services that your practice provides that you may not be fully maximizing as a billing opportunity. It is important that you are submitting and receiving reimbursement for the hard work your providers are completing to support your patients.
Objectives
In this course, you will learn the requirements for advanced coding opportunities including Transition Care Management, Chronic Care Services, Advanced Care Planning, and Prolonged Services. Thus, providing you with the insight needed to maximize your revenue potential without increasing your workload. Upon completion of this course, you will be able to do the following:
  1. Describe the regulatory compliance requirements for Transitional Care Management, Chronic Care Services, Advanced Care Planning, and Prolonged Services.
  2. Identify implications at the practice and the individual provider level that must be considered when implementing the described codes and services.

In order to earn credit for participation in this activity, the learner must complete a 5-question post-test with a passing score of 80% or better with unlimited attempts.
Intended Audience
This course is intended for any house call providers, practice manager, and coding staff.
