HCCI House Call Practicum: Telemedicine Elective
Description
The HCCI House Call Practicum™ Telemedicine Elective is a full-day activity that can be added onto the 2-Day Practicum experience or is a stand alone one day activity designed to introduce you to the various technologies used by Cleveland Clinic to conduct virtual visits with their patients through synchronous, bi-directional video with the assistance of paramedics at the home.  You will first receive an overview of telemedicine and remote patient monitoring via the HCCI eLearning module, Telehealth in Home-Based Primary Care. Next, you will ride along with the paramedics to observe the use of the video technology at the patient’s bedside. Finally, you will spend time with the provider back at the office, observing several virtual visits, discussing the various factors involved in implementing a telemedicine program, including potential barriers, privacy legal issues, selection of appropriate technology, patient/caregiver satisfaction, and reimbursement considerations.

Your two-day experience at the Cleveland Clinic’s Medical Care At Home Program will provide you with a front row seat to the clinical and practice management operations housed at this large multispecialty nonprofit academic medical center in Ohio. Cleveland Clinic’s Medical Care At Home Program provides primary care services to more than 1,700 frail, home-limited seniors who would otherwise struggle to be cared for in a traditional office setting. Cleveland Clinic’s Medical Care At Home Program also participates in two alternative payment models, CPC+ and Independence at Home, which enables them to be paid on outcomes and value rather than typical fee-for-service contracts. Cleveland Clinic’s Medical Care At Home Program includes physicians, nurse practitioners, a physician assistant and nurse manager, medical assistants, paramedics, care coordinators, patient services specialists, a medical coder, a reimbursement specialist, and more. In addition to ongoing primary medical care and management of chronic illness, the program offers geriatric consults (for evaluation of memory, physical and mental capacity, fall risk and more), palliative consults, and telemedicine for urgent visits.
Objectives
Upon completion of the HCCI House Call Practicum Telemedicine Elective, you will be able to:

1. Describe factors to consider prior to implementation of a telemedicine program

2. Review how technology is used in the home

3. Explain the staffing of the telemedicine program

4. Discuss how patients are scheduled for virtual visits
Intended Audience
This course is intended for physicians, advanced practice providers, nurses, practice managers/administrators, social workers, residents/fellows, medical assistants, and other practice operations staff.

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of PeerPoint Medical Education Institute and the Home Centered Care Institute.
