Description
The Advanced Applications of Home-Based Primary Care™ two-day workshop will be presented virtually on June 24-25, 2021 and is designed for experienced HBPC providers and practice staff. Case-based sessions focus on higher-level topics in HBPC such as advance care planning, prognostication, care coordination and transitions of care, caregiver support and challenging clinical issues such as dementia and polypharmacy. Instruction will also include video simulation of procedures commonly performed in the home.

In addition, this workshop features tips and strategies related to coding and documentation, workflow and staff productivity, and demonstrating the value of HBPC. You will leave this interactive workshop with enhanced knowledge and skills you can immediately and confidently apply in your practice. 
Intended Audience
This workshop is intended for physicians, advanced practice providers (NPs, PAs), nurses (RNs, LPNs), practice managers/administrators, social workers, residents/fellows, medical assistants, and other practice operations staff.
Objectives
Upon completion of the workshop, learners will be able to:

1. Discuss optimal care and management strategies through challenging home-based primary care (HBPC) cases, including patients with dementia.

2. Assess evidence-based practices for medication management, identify potential pitfalls and dangers of polypharmacy, discuss a structured approach for de-prescribing and review strategies for maximizing adherence.

3. Apply optimal practices in coordinating the care of homebound patients during transition, including handoffs, medication reconciliation and collaboration with community agencies.

4. Manage difficult conversations with patients and family around advance care planning, serious illness communication and prognostication. 

5. Describe the importance of caregiver support, as well as the screening tools and resources that can be utilized for caregivers.

6. Identify billing opportunities and documentation requirements beyond the normal E/M codes and apply strategies for implementing these services into daily practice.

7. Compare various staffing models for HBPC and examine opportunities for improving workflow and efficiency.

8. Describe the steps involved in performing common procedures required in HBPC, including tracheostomy tube exchange, gastrostomy tube removal and replacement, wound care, and knee joint aspiration and injection.

Cancellation Policy
Maximum Workshop Enrollment 
Workshop enrollment is limited to 40 learners per offering. If paying by check, your registration is not considered final until payment has been received and your registration confirmed by HCCI.
 

Cancellation & Refunds 
HCCI reserves the right to cancel a Workshop at any time for any reason. If HCCI cancels a Workshop, you will have the option to either a) transfer tuition paid to another HCCI Workshop or b) receive a refund.You must inform HCCI of your choice of either the transfer or refund within two (2) weeks of the Workshop cancellation. If no such notification is received, HCCI will issue a refund.
 

If you wish to cancel your registration you must send notice of cancellation via e-mail to education@hccinstitute.org. Notices received prior to 5:00 pm CST two weeks prior to the first day of the Workshop will be refunded the amount of the tuition paid less a $75 administrative fee. In lieu of a refund, the full amount of the tuition paid may be applied to a future HCCI Workshop. The request to apply tuition credit to a future Workshop must be made in writing by e-mail within two weeks of the cancellation or a refund will be issued.
   

Cancellation requests submitted after the deadline noted above, will not receive a refund. No-shows will not be eligible for a refund or transfer of tuition.  

Other 

Requests to transfer your registration to another person will be considered on a case-by-case basis. Transfer requests must be made in writing no later than 5:00 pm CST one week before the first day of the Workshop. The Workshop is designed as a comprehensive two-day activity, and enrollment may not be “split” between participants. Likewise, there is no single-day tuition. 

HCCI is not liable or responsible for any travel, lodging or work expenses incurred in the event of a Workshop cancellation that is initiated by either HCCI or the learner.
