Objectives
  • Review the coding, reimbursement, and policy impacts specific to home based medical care as a result of the CY 2024 Medicare Physician Fee Schedule Final Rule (MPFS) and how to best respond to these changes.
  • Discuss payment and documentation requirement impacts to the home and domiciliary Evaluation and Management (E/M) codes and identify alternate reimbursement opportunities.
  • Call out changes to home based medical care management services, including Chronic Care Management, Transitional Care Management, Telehealth, Behavioral Health, and Advance Care Planning and use case-based examples to understand how to maximize reimbursement.
  • Provide clarification on risk adjustment models, including Hierarchical Condition Category (HCC) Coding.
