Virtual Boot Camp in Home-Based Primary Care: Essential Elements and Advanced Applications in Home Based Primary Care™
Special Promotion
Since participants often leave the workshops energized and eager to implement what they’ve learned, we are pleased to include in your Virtual Boot Camp enrollment a free post-workshop, one-hour consultation with our experts (limited to one free consultation per practice). You will be invited to bring into this discussion others from your practice to expose them to some of the clinical and operational enhancements introduced at the workshops, and we will propose some preliminary steps to help you achieve your goals.
Description
The Essential Elements in Home-Based Primary Care™ two-day foundational workshop will be presented virtually on April 15-16, 2021 and is designed for learners who are considering or who are relatively new to offering home-based primary care (HBPC) services for patients with complex medical conditions. Through this workshop, clinical providers, practice managers and operations staff will enhance their knowledge, competence, and performance in providing optimal HBPC based on current recommendations from national experts. This virtual workshop features case-based sessions focusing on clinical and economic factors unique to home-based primary care, including patient assessment; infection control; medication management; acute/urgent care; technology useful in enhancing patient care and productivity; cultural competence; staffing, operations and durable medical equipment; coding and billing; provider safety and self-care.

The Advanced Applications in Home-Based Primary Care™ two-day workshop will be presented virtually on June 24-25, 2021 and is designed for experienced home-based primary care (HBPC) providers and practice staff. Case-based sessions focus on higher-level topics in HBPC such as advance care planning, prognostication, care coordination and transitions of care, caregiver support, and challenging clinical issues such as dementia and polypharmacy. Instruction will also include video simulation of procedures commonly performed in the home.
