Diversity, Equity, and Inclusion for Home-Based Care
Description
Caring for patients in their home gives a perspective not typically available in office-based or hospital settings and being able to competently leverage this enhanced understanding of patients and their families can lead to improved healthcare experiences. Health systems are on the front lines of addressing health equity, but the topic is complex and multifaceted. Organizations need to be thoughtful in how they address patient cultural diversity and/or issues related to systemic inequity and exclusion. Leaders must be deliberate in designing and building systems where the outcomes truly advance health equity for all patients and families.
Objectives
In this course, you will learn how to develop and implement best practices for culturally sensitive care in the home environment and within the workplace, including the use of organizational self-assessment and diversity, equity, and inclusion (DEI) reference tools. Upon completion, you will be able to do the following:
1. Define the importance of assessing and promoting DEI practices for your house call program.
2. Identify and develop clinical and operational best practices for culturally sensitive home-based medical care.
3. Incorporate DEI and cultural competence principles into a patient-centered plan of care.
4. Implement a formal DEI internal initiative for recruitment, representation, performance management, and retention within your house call program.

In order to earn credit for participation in this activity, the learner must complete a 5-question post-test with a passing score of 80% or better with unlimited attempts.
