The CAPC Payment Accelerator: A Joint Virtual Workshop for Home-Based Care Teams

Co-sponsored by the Center to Advance Palliative Care (CAPC) and the Home Centered Care Institute (HCCI), CAPC Payment Accelerator: A Joint Virtual Workshop for Home-based Care Teams helps organizations that specialize in the care of high-need, seriously ill patients to understand and pursue value-based payment opportunities. Join us to learn how to build sustainable financial partnerships with health insurance plans, accountable care organizations, and others—both to secure initial support and operate effectively within value-based contracts. 

Note: New users of the HCCI Learning Hub will be asked to complete a one-time registration prior to signing up for this workshop.
Intended Audience

CAPC Payment Accelerator: A Joint Virtual Workshop for Home-based Care Teams is open to all clinical care programs serving high-need patients in the home setting, which may include long-term care settings. For maximum benefit, HCCI and CAPC recommend that teams of 2-3 individuals from the same program or organization attend the workshop together, including the following roles: 

  • Clinical program leadership – responsible for program design and the quality of care delivered
  • Financial leadership – responsible for program budgeting and/or revenue cycle 
  • Marketing/business development leadership – responsible for external relationships and growing referrals to the program 
  • Contracting/managed care leadership – responsible for building relationships with payers and/or negotiating payment contracts 
Note: A discounted rate of $145 per attendee is available for teams of 3 or more from the same practice/organization. Please contact HCCI at education@hccinstitute.org  for additional information or to enroll 3+ team members at the discounted rate. 
Objectives

Upon completion of the workshop, learners will be able to:

  1. Acquire a working knowledge of value-based payment contracts, including common quality measures and payment model alternatives; 

  1. Identify potential partners in their local areas, and hone their approach to negotiating and building relationships; 

  1. Use financial information to assess gaps and propose prices; 

  1. Integrate new strategies for program efficiency, including staffing models, telehealth, population stratification and management, community health workers, and coordination with other providers.

Enrollment is limited to 200 learners. If paying by check, your registration is not considered final until payment has been received and your registration confirmed by HCCI.  

Changes, Cancellations, and Refunds  

HCCI and CAPC reserve the right to modify the schedule and faculty assignments as needed.  

HCCI and CAPC reserve the right to cancel this workshop at any time for any reason.  If this activity is cancelled, registrants will receive a refund within two (2) weeks of receiving notification of the cancellation. 
 
If a registrant wishes to cancel their registration, they must send notice of cancellation via e-mail to education@hccinstitute.org. Notices received prior to 5:00 pm CST two weeks prior to the Workshop will be refunded the amount of the tuition paid, less a $35 administrative fee. 
 
Cancellation requests submitted after the deadline noted above will not receive a refund. No-shows will not be eligible for a refund.
