Description
Comprehensive medication management is a key component of care for older home-limited patients and can be one of the most challenging and time-consuming activities. Nearly 20% of community dwelling adults age 65 and older take 10 or more medications. Multiple medication use is associated with increased non-adherence, side effects, and financial burden. Several of the medications may be prescribed inappropriately, and the lack of clear evidence-based therapeutic guidelines for home-based primary care (HBPC) patients with multimorbidity and disability further complicates medication management.
Objectives
In this course, you will identify the challenges faced by both HBPC providers and patients and apply the essential steps to manage medications for home-based patients. Upon completion, you will be able to do the following:
- Recognize the benefits and challenges providers face when managing medications for home-limited and HBPC patients.
- Explain the issues that home-limited and HBPC patients face when trying to adhere to a prescribed medication regimen.
- Apply a medication management model to prescribe a safe and effective regimen that patients can follow.

In order to earn credit for participation in this activity, the learner must complete a 5-question post-test with a passing score of 80% or better with unlimited attempts.
Intended Audience
This course is intended for primary care providers working with homebound patients, including physicians, nurse practitioners, physician assistants, and other health professionals.
