When this site is selected, the HCCI House Call Practicum™ gives you behind-the-scenes access to the clinical and practice management operations housed at this large multispecialty nonprofit academic medical center. Located in Independence, OH, the Cleveland Clinic Medical Care at Home program provides primary care services to more than 2,100 frail, home-limited seniors who would otherwise struggle to be cared for in a traditional office setting. The program includes physicians, nurse practitioners, physician assistants, community paramedics, and a nurse manager, medical assistants, coordinators, patient services specialists, a medical coder, a reimbursement specialist, and more. In addition to ongoing primary medical care and management of chronic illness, the program offers geriatric consults (for evaluation of memory, physical and mental capacity, fall risk and more) and telemedicine for urgent visits. Cleveland Clinic Medical Care at Home is also integrated with the primary care practices at Cleveland Clinic and engages in a variety of novel care models to care for patients and achieve value based goals.
