Care Management for Homebound Patients Online Course
Description
High-risk or chronically ill patients often require care from multiple individuals, including home-based primary care (HBPC) teams. Providing safe, high-quality care that puts the patients’ needs first requires thoughtful and strategic care management and coordination among HBPC team members.
Objectives
In this course, you will learn how to support and develop best practices for meeting the individual needs of patients through care management and coordination.

Upon completion, you will be able to do the following: 

1.Define care management and care coordination in home-based medicine. 

2. Identify gaps in care, and target care management strategies to proactively manage complex populations 

3. Describe interdisciplinary team member roles in home-based medicine and strategies to engage in top-of-licensure work. 

4. Utilize care management tools and interventions to support proactive care. 

Intended Audience
This course is intended for members of the clinical team caring for homebound patients, including nurses and social workers.
Estimated Time to Complete
30 minutes
In order to earn credit for participation in this activity, the learner must complete a 5-question post-test with a passing score of 80% or better with unlimited attempts.
