Description
In healthcare, teamwork is vitally important to the delivery of high-quality patient care. Ineffective communication and collaboration are responsible for up to 70% of the adverse events reported in healthcare. The home-based primary care (HBPC) team faces unique challenges that make effective teaming more difficult than in other healthcare providing teams, potentially leading to medical errors and adverse events.
Objectives
In this course, you will learn best practices to build an environment that promotes team cohesiveness and open communication to better support your patients and their families. Upon completion, you will be able to do the following:
Describe the characteristics of an interdisciplinary HBPC team and contrast with characteristics of a multidisciplinary team.
Identify barriers to developing strong interdisciplinary HBPC teams.
Apply best practices to improve HBPC team effectiveness.

This course is intended for any healthcare professional who works with a team to provide home-based primary care (HBPC) services to patients.
