The Home Centered Care Institute (HCCI) and the National Partnership for Healthcare and Hospice Innovation (NPHI) are collaborating to help hospice and palliative care centers create sustainable, innovative house call programs that enhance the patient experience, improve health outcomes, and reduce costs.
