Other questions address the ability to obtain a timely house call, the courtesy and caring of the office staff, the quality of care provided overall, and any additional feedback the patient may have.<br /><br />The survey encourages patients to share any comments or suggestions they may have and provides contact information for the office where the survey can be returned. The survey was developed by the Home Centered Care Institute with the goal of continuously improving the quality of care provided to patients and their caregivers.
