Home-Based Primary Care: A Powerful Solution for Home-Limited, Medically Complex Patients - 2024

Home-Based Primary Care (HBPC) is a service that provides primary medical care to medically complex patients in their own homes. It is often referred to as the "modern-day house call." HBPC is delivered by physicians, nurse practitioners, and physician assistants who travel to the patient's home to deliver care. These providers may also work as part of a team to address both medical and non-medical needs of the patient. HBPC has been shown to improve health outcomes, enhance quality of life, and reduce costs by preventing unnecessary emergency room visits and hospitalizations.<br /><br />HBPC includes a range of services that are typically provided in a primary care office. These services can include routine medical care, management of chronic diseases, wellness visits, urgent medical care, vaccinations, management of cognitive and neurological disorders, wound care, coordination with specialists and diagnostic testing, medical visits at assisted living facilities or other community settings, and caregiver support.<br /><br />HBPC is typically covered by Medicare for eligible beneficiaries. Some managed care or health insurance plans may also cover HBPC, but may require a referral or designation of the HBPC provider as the primary care provider. To determine if HBPC is right for a patient, factors such as difficulty leaving the home, need for special accommodations or assistance, adverse effects on health from leaving the home, and availability of caregivers or family members for transportation should be considered.<br /><br />If HBPC seems appropriate, patients should discuss it with their primary healthcare provider who may be able to refer them to an HBPC provider. Other in-home services available include skilled home health services, concierge healthcare, hospital at home, palliative care, and hospice care.
