The main focus was on leveraging a state-level approach to boost awareness and access to Home-Based Primary Care (HBPC) in Florida.<br /><br />**Objectives & Project Goals:**<br />The webinar aimed to discuss insights from the Florida House Call Project, evaluate research findings on HBPC awareness, identify gaps and unmet needs, and review early project outcomes. The project sought to expand the national reach of home-centered care, enhance demand and capacity, undertake statewide research, and raise provider and public awareness of HBPC.<br /><br />**Role of Champions:**<br />HBPC Champions act as state and national leaders, spreading awareness through media and public forums while utilizing HCCI resources. They are responsible for educating providers and operations staff locally.<br /><br />**Champion Training & Activities:**<br />A cohort of twelve champions, including physicians, nurse practitioners, and business managers, participated in various training activities such as online courses, forums, and workshops. Their activities included guest speaking, virtual mentorship, media interviews, and preceptorship, which contributed to increased referrals, additional patients, enhanced clinical competence, and favorable media attention.<br /><br />**Research Insights:**<br />Surveys conducted in 2021 revealed key insights. A consumer survey showed that perceptions of "house call" and "home-based primary care" were largely positive, with mobility issues significantly influencing these perceptions. For providers, a survey highlighted the interest in learning more about HBPC and identifying home-limited patients. It also created profiles of patient characteristics, payer mixes, and referral sources, showing that more than half of their patients are homebound or have limited mobility.<br /><br />**Supply and Demand:**<br />Research indicated the supply distribution in Florida, summarizing the number of providers, sites, and annual visits. It also shed light on partnerships with home health and hospice agencies to meet high demand.<br /><br />**HCCIntelligence Resource Center:**<br />The resource center provides hotline support, webinars, virtual office hours, and downloadable tools related to HBPC, funded partly by The John A. Hartford Foundation.<br /><br />This initiative aims to increase HBPC's footprint in high-need areas by enhancing the knowledge base, expanding resource networks, and establishing effective patient care models.
