Shared Decision Making (SDM) is described as a collaborative process between clinicians and patients to align decisions with the patient's values.<br /><br />The approach includes several stages of ACP tailored to individuals at different points in their healthcare journey, from those newly engaged in planning to those with serious illnesses making current healthcare decisions.<br /><br />To implement these concepts, five promises include initiating conversations, providing decision-making assistance, ensuring clear plans, updating and using plans, and honoring preferences and decisions. Dr. Montgomery stresses that a conversation alone is insufficient; a supportive system is essential for quality outcomes.<br /><br />Elements for a sustainable program encompass system redesign, education and certification, community engagement, and continuous quality improvement. Success in these areas can contribute to a transformative healthcare culture that genuinely reflects person-centered, family-oriented care.
