Respecting Choices (RC) is a program that aims to support organizations and communities in honoring and respecting individuals' preferences and decisions through person-centered care. Person-centered decision making is the central focus of this approach, which involves actively engaging and supporting individuals on their decision-making journey.<br /><br />RC has two programs, Advance Care Planning (ACP) and Shared Decision Making in Serious Illness (SDMSI), which work together to embed person-centered decision making as a standard. These programs integrate with each other and use consistent language, strategies, and processes across various care settings.<br /><br />The implementation of the RC program has resulted in improved outcomes. Some of the evidence-based outcomes include individualized planning discussions, provision of care consistent with patient goals and values, clear and available ACP plans for healthcare providers, increased patient and family satisfaction with ACP conversations, decreased decisional conflict, increased surrogate understanding of patient's goals of care, increased congruence in patient and surrogate decisions, and a positive impact on family members through reduced stress, anxiety, and depression in surviving relatives.<br /><br />Respecting Choices has over 20 years of experience working with healthcare organizations and communities. Some of the experiential conclusions they have drawn include improved clinician competency and comfort level with conversations, clarification of patient goals of care, a shift from crisis end-of-life decision making to early and effective planning conversations, translation of patient goals and decisions into written plans, specific guidance in clinical decision making, timely referrals for other services, decreased moral distress for healthcare providers, and a standardized patient and clinician experience.<br /><br />Respecting Choices has successfully implemented its model in various settings, from small hospitals to large health systems, and has had a positive impact on diverse communities. The program has led to increased prevalence of planning in racially, ethnically, and culturally diverse communities, increased hospice use at the end of life, and improved hospital CPR success rates.<br /><br />Overall, the Respecting Choices program offers a system for person-centered decision making that has proven to transform healthcare by improving outcomes and promoting individual preferences and values.
