Pdf Summary
The HCCIntelligence™ Webinar on August 19, 2020, focused on integrating Occupational Therapy (OT) into Home-Based Primary Care (HBPC). The session featured Monika Robinson, DrOT, from Midwestern University and Sarah Szanton, PhD, from Johns Hopkins School of Nursing. Their main objectives were to delineate OT's role in HBPC, demonstrate how OT addresses the Geriatric 5Ms areas of care and quality metrics, and illustrate successful models of home-based care such as the CAPABLE project.

The role of OTs in HBPC entails assessing the person-environment-occupation fit, using activity analysis, and providing compensatory and adaptive equipment knowledge. OTs work comprehensively across various health conditions, focusing on function and participation through longitudinal care rather than episodic or post-acute interventions.

Primary care OT differs from traditional OT rehab by emphasizing the primary prevention of health issues and providing continuous support for patients' ongoing health needs. Significant evidence supports the efficacy of OT in improving health and function, particularly for older adults and those with chronic conditions, often proving cost-effective in primary care settings.

The CAPABLE project is a notable OT-driven model aiming to improve elder health outcomes by aiding them in aging in place. CAPABLE focuses on enhancing physical function, reducing depression, and decreasing hospitalizations and nursing home admissions by combining interventions from OTs, registered nurses, and handymen. The project's success is evidenced by improved activity levels, reduced depressive symptoms, and significant Medicaid savings.

In conclusion, the integration of OT into HBPC, supported by models like CAPABLE, demonstrates profound beneficial outcomes, including enhanced patient autonomy, reduced healthcare costs, and improved quality of life for elderly individuals.
