The "Essential Elements of Home-Based Primary Care" (HBPC) workshop, held virtually on June 2-3, 2022, is aimed at learners new to providing HBPC services to patients with complex medical conditions. This interactive, foundational two-day course focuses on imparting essential knowledge and skills through case-based sessions and covers a wide range of topics critical for effective HBPC. <br /><br />Key areas include patient assessment, prognostication, advance care planning, medication management, acute and urgent care, supportive technologies, caregiver support, cultural competence, operational workflows, budgeting, revenue management, coding, billing, and provider safety and self-care. <br /><br />Upon course completion, participants will be able to accurately describe HBPC principles, apply the "4Ms" in older adult care, manage various clinical care aspects for homebound patients, demonstrate optimal patient care in home settings, recognize and address caregiver burnout, and understand the business and economic dynamics of HBPC. They will also examine effective staffing models and opportunities for workflow enhancements.<br /><br />Day One of the workshop features presentations and hands-on sessions addressing foundational HBPC principles, patient assessments, quality care transitions for homebound patients, and case studies that investigate specific patient scenarios. Topics covered include prognostication, advance care planning, managing multiple health conditions, acute/urgent care needs, medication alignment, mental health management, and operational aspects such as coding, documentation, billing, and economics of HBPC.<br /><br />Day Two continues with additional patient case studies, discussions on cultural competence, medical decision-making capacity, workflow improvements, safety strategies, and community resource collaboration. This day also includes sessions on budgeting, marketing, staffing, and a simulated house call exercise. <br /><br />The workshop culminates with insights into the future of value-based healthcare in the home and ends with a Q&A session.<br /><br />The event is supported by The John A. Hartford Foundation and faculty members include experts from diverse healthcare backgrounds such as Dr. Paul Chiang, Megan Verdoni, and Brianna Plencner, among others.
