Discuss virtual visits and RPM, addressing technology infrastructure and reimbursement requirements.<br /><br />### Agenda:<br />1. **Introduction** (5 min): Discuss current experiences with telehealth and potential continued use.<br />2. **Barriers** (5 min): Identify and strategize solutions for challenges to telehealth adoption, including policy limitations, patient technology issues, and WiFi access.<br />3. **Virtual Visits** (10 min): Consider challenges and best practices for virtual visits, including the collection of clinical information and appropriate visit types.<br />4. **Remote Patient Monitoring (RPM)** (5 min): Explore the clinical advantages of RPM and the necessary workflow adjustments.<br />5. **Implementation Considerations** (15 min): Discuss integrating telehealth into practice, including scheduling, workflow documentation, evaluation methods, and vendor questions.<br />6. **Debrief** (5 min): Review Next Steps handout and share implementation ideas.<br /><br />### Discussion Highlights:<br />- **Introduction**: Invite participants to share their telehealth experiences and consider chronic diseases for RPM feasibility.<br />- **Barriers**: Focus on solving barriers specific to telehealth, with practical solutions such as patient/caregiver education support.<br />- **Virtual Visits**: Analyze information collection differences between in-person and virtual visits, supported by a review of telehealth CPT and HCPCS codes.<br />- **RPM**: Discuss clinical examples and workflow adjustments for effective RPM integration.<br />- **Implementation & Evaluation**: Plan for telehealth continuation, workflow changes, and telehealth effectiveness evaluation metrics.<br /><br />Facilitators are prompted to capture group discussions and ideas on a flipchart for future reference and encourage group brainstorming or small group breakout discussions to enhance participation and outcomes.
