Pdf Summary
The "Crossroads of Care: Managing Serious Illness in the Home" session held in Schaumburg, Illinois, on June 13, 2023, aims to advance home-based primary care to ensure that medically complex patients receive high-quality care at home. The event was organized by the Home Centered Care Institute (HCCI), which is involved in education, consulting, research/analytics, and advocacy to support home-based medical care.

The session introduced resources available through the HCCIntelligence Resource Center, including a hotline, webinars, and downloadable tools and guides focusing on home-based medical care topics. Sponsored in part by the John A. Hartford Foundation, these resources are intended solely for educational purposes and not as medical advice.

Key session topics included the integration of Home-Based Primary Care (HBPriC) and Home-Based Palliative Care (HBPalC) to offer a comprehensive solution for managing serious illnesses and chronic conditions. Objectives were set to describe the unique and aligned aspects of these care models, discuss the benefits of integrating them, and identify strategies for successful implementation.

Dr. Paul Chiang, the Medical Director of Northwestern Medicine HomeCare Physicians, spoke on the advantages of HBPriC, such as improved quality of life, prevention of hospitalizations, and cost reductions. HBPriC's multiple forms, clinical care models (transitional and longitudinal care), and patient characteristics were also discussed, illustrating the model's adaptability to various healthcare settings.

The importance of technology, workforce competencies, and the 4Ms framework in caring for older adults was highlighted. Christina's case study demonstrated the practical implications of integrating HBPriC and HBPalC for a full-service solution in managing complex cases.

Future sessions are planned on topics such as optimizing house call operations and payer contracting to demonstrate home-centered care’s value. The event also recognized the need for more home-based care providers and showcased the successful outcomes of such integrated care models, particularly in palliative scenarios.

Finally, the session offered free online courses to further education in home-based care and concluded with a networking reception.
