The Advanced Applications of Home-Based Primary Care two-day workshop was held at Dallas Marriott Suites Medical/Market Center from December 1-2, 2022. It aimed to equip experienced home-based primary care (HBPC) providers and practice staff with strategies to succeed in value-based payment arrangements and to advance their knowledge. The workshop covered essential topics including required staffing, infrastructure, and data; patient identification; cost-effective management of common HBPC conditions (e.g., dementia, congestive heart failure); quality measurement and reducing care gaps; risk adjustment models like HCC coding; and negotiating with payers. Participants also engaged in hands-on training for HBPC procedures such as wound care, gastrostomy and tracheostomy tube changes, joint injections, and Point-of-Care Ultrasound (POCUS).<br /><br />Key learning objectives were:<br />- Discussing available value-based payment opportunities for HBPC programs.<br />- Applying recommended practices for staffing, productivity, telehealth, risk stratification, and provider coordination.<br />- Managing specific conditions in value-based payments effectively.<br />- Handling transitions of care for homebound patients with a focus on various domains.<br />- Performing common HBPC procedures.<br /><br />Faculty members featured in the workshop included Paul Chiang, MD, James Ellison, MD, MPH, Michael Helle, NRP/CCP, FP-C, MHA, MBA, Daniel Kim, MD, and Amanda Tufano, MHA, FACHE, FACMPE, among others.<br /><br />In addition, the workshop provided practical resources such as checklists and step-by-step guides for various HBPC procedures and encouraged participants to continue their professional development through continued medical education credits. The Home Centered Care Institute spearheaded the event and emphasized its non-endorsement policy regarding specific healthcare products or providers. The workshop acknowledged support from The John A. Hartford Foundation.
