A Prescription for Note Bloat: An Effective Progress Note Template Research

The study examines the impact of an educational session bundled with a progress note template on note quality, length, and timeliness in four academic hospitals in the United States. The intervention consisted of educational lectures and the implementation of an electronic progress note template. The results showed that the bundled intervention significantly improved the quality of progress notes, decreasing their length and resulting in earlier completion. The use of the note template was associated with higher quality scores, shorter notes, and earlier completion time. The intervention also improved documentation of only relevant lab values and studies, the presence of a discharge plan, and a relevant and focused physical exam. However, there were variations between institutions in terms of the percentage of template use and improvements in note quality. The study suggests that a progress note template combined with educational interventions can lead to more accurate and succinct progress notes, improve efficiency, and potentially improve patient care. Further research is recommended to validate the results and to explore the long-term impacts of these interventions.
